Despite bundled payments initiatives and ACO approaches, health care operates in a silo. That is no more apparent and important than in the care of our elders. What happens during transitions of care and in the eighty percent of care that is impacted by social determinants of health is more important than the twenty percent of care in hospital and practice settings. 

In this keynote, aging expert and ACHE Fellow Anthony Cirillo will connect the dots in aging care. As a caregiver to his mom Philomena, Anthony came full circle in his health and aging journey. His caregiving journey began after his sister passed, a not uncommon occurrence among family caregivers. At age 91, he moved his mom from Florida to care for her in North Carolina.
