Niall Downey brings a rare and compelling perspective shaped by two high-stakes careers—medicine and aviation. Trained as a surgeon before becoming a long-haul airline captain, he has spent decades understanding how and why things go wrong. Today, he helps organisations rethink safety, human error, and accountability by translating proven aviation principles into practical strategies for healthcare and beyond. Niall Downey – Bridging Aviation and Healthcare Safety

Niall Downey qualified as a doctor from Trinity College Dublin in 1993 before training as a surgeon in Belfast, where he received his FRCSI in 1997. His early career included working in cardio-thoracic surgery as an SHO in the Royal Victoria Hospital in Belfast. He later returned to Dublin, taking on registrar roles in the National Cardiac Surgery Unit in the Mater Hospital and at Our Lady’s Children’s Hospital in Crumlin.

Despite a successful medical career, Niall made a bold transition in 1999, retraining as an airline pilot with Aer Lingus. For six years, he uniquely combined both professions, continuing to work as an Accident & Emergency doctor while building his aviation career. This dual experience gave him a deep understanding of decision-making, pressure, and human performance in critical environments.

 

From Co-Pilot to Captain

After flying both European and Trans-Atlantic routes, Niall qualified as a captain in 2010. He now operates from Manchester on the Airbus A330 Trans-Atlantic fleet. His aviation career has provided extensive insight into structured safety systems, teamwork, and communication—elements that are often less developed in other industries.

This combination of medical and aviation expertise forms the foundation of his work today, allowing him to translate lessons across sectors in a way that resonates with diverse audiences.

 

Framework Safety Group Ltd

In 2011, Niall founded Frameworkhealth Ltd, a company designed to bring aviation-style safety training into healthcare. Built on more than thirty-five years of combined experience, the initiative focuses on reducing adverse events—many of which stem from systemic issues rather than individual mistakes.

The company has since evolved into Framework Safety Group Ltd, reflecting growing demand from industries beyond healthcare. The mission is clear: to share aviation’s Safety Management System and embed a systemic Human Factors approach across organisations.

Niall challenges the traditional tendency to blame individuals and instead encourages a broader understanding of how systems, culture, and human limitations interact. His work promotes the adoption of a Just Culture, where people feel empowered to speak up without fear, ultimately improving outcomes and reducing risk.

 

Speaking, Media and Thought Leadership

Niall Downey is a highly experienced speaker who has presented at conferences locally, nationally, and internationally. His ability to combine storytelling with practical insights makes his sessions both engaging and impactful.

He has contributed articles to specialist journals and national publications, including The Irish Times. His expertise has also been featured on national television such as RTE TV News and Virgin Media, as well as multiple appearances on BBC Radio.

In 2016, following a widely praised TEDx talk in Stormont’s Great Hall, Niall was appointed as an Expert Advisor to the Northern Ireland Executive’s Department of Health—recognition of his influence in shaping safety thinking at a strategic level.

 

Author of ‘Oops! Why Things Go Wrong’

In 2023, Niall published his first book, Oops! Why Things Go Wrong. The book explores the universal nature of error across industries and society, while offering practical ways to address it. Its success, including a second print run driven by strong demand, has expanded Niall’s reach beyond healthcare into a wide range of sectors.

The book reinforces his core message: human error is inevitable, but systems can be designed to manage and reduce its impact.

 

Beyond the Stage

Earlier in his career, Niall competed in cycling at national level for thirteen years and at pro-am international level in the early 1990s. He was part of Northern Ireland’s Commonwealth Games panel and later contributed to the sport as a Medical Officer for both the Federation of Irish Cyclists (now Cycling Ireland) and the Ulster Cycling Federation. He also worked internationally as a Race Doctor for six years.

These experiences further shaped his understanding of performance, teamwork, and resilience under pressure.

Error is Inevitable, Poor Outcomes are Optional. Lessons from the Flightdeck

Humans have limitations. Our brains operate on a two speed mode, fast and slow. The older part of the brain operates mainly in survival mode so kicks in quickly and operates on ‘Fight or Flight Mode’ which has a wide margin of error, often outside the margin of safety in our complex world. 

This is why errors are inevitable and often, very costly. There are techniques however which help the newer, higher functioning parts of the brain to over-ride the immediate response allowing a narrower margin of error (and better safety margins) although at the cost of higher energy consumption, a very good trade-off in our modern society. 

Niall simplifies the neuro-anatomy and evolutionary psychology into usable tools and techniques which help us achieve this. His keynote brings fifty years of aviation lessons out of the Flightdeck and into your industry.

Flying on Autopilot - And Why it’s a Bad Idea

Human brains have evolved over millennia to be very efficient, important given the hugely disproportionate amount of energy they consume relative to their size. Unfortunately, part of the deal is that much of the work is delegated to the older sections of the brain, the mid-brain which uses pre-determined pathways to react quickly, generally to simply keep you safe. The message often doesn’t get escalated to the newer and much more powerful areas of the brain, the fore-brain or neo-cortex which can analyse and come up with much better (but costly in terms of energy) solutions to the situations we find ourselves in. 

This leads to poorer outcomes for everyone involved due to adverse events, increased costs, higher stress levels and burn-out. 

Niall outlines how aviation has addressed these problems, overcome the evolutionary tripwire and shows how the techniques are relevant and transferable to other industries improving outcomes for staff, clients and your company alike.
